Your journey begins with a thorough health assessment. This includes:
- A pre-appointment questionnaire
- Detailed medical history review
- Physical examination
- Blood tests inc. Urea & Electrolytes, Liver Function Tests, Pancreatic Amylase and HbA1c
This comprehensive evaluation allows us to understand your health status and develop a treatment plan suited for you.
Ongoing Support and Monitoring
We believe that support is key to successful weight loss. Our team provides:
- Regularly scheduled check-ins with our GPs and nurses to monitor your progress
- Adjustments to your plan based on your needs and progress
- Support and encouragement to keep you motivated
Patient Care Pathway
Initial Consultation
(GP Consultation)
- GP review of your pre-appointment questionnaire
- Medical history review
- Blood pressure, height, weight, BMI, waist circumference
- Blood tests to check Urea & Electrolytes, Liver Function, Pancreatic Amylase and HbA1c
3 Month Follow-Up
(Nurse Consultation)
- The practice nurse will check weight, BMI, waist circumference, blood pressure
- Report any issues or side effects to the GP
- Arrange for the GP to issue a repeat prescription if needed
6 Month Follow-Up
(GP Consultation)
- GP review of your progress to date
- Blood pressure, weight, BMI, waist circumference
- Blood tests to check Urea & Electrolytes, Liver Function, HbA1c and Pancreatic Amylase
- Issue a repeat prescription is needed
9 Month Follow-Up
(Nurse Consultation)
- The practice nurse will check weight, BMI, waist circumference, blood pressure
- Report any issues or side effects to the GP
- Arrange for the GP to issue a repeat prescription if needed
